Community Health Worker (FT) - Integrated Care for Adults (InCA)

Minimum Requirements:
- High School Diploma or equivalent with any combination of 3 years health/social services experience.
- Experience in engaging community members.
- Excellent computer skills, task oriented.
- Preference will be given to candidate with experience working with clients with psychiatric or substance use issues and complicated medical needs.
- Must have valid driver’s license, transportation and evidence of insurability.
Job Duties:

- Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
- Coach clients in effective management of their chronic health conditions including mental health and substance use disorder.
- Assist clients in developing and understanding care plans.
- Document activities, care plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
- Work collaboratively and effectively within a team.
- Make contact with disengaged attributed clients via telephone, mail, and face to face interactions in the home/community.
- Uploads appropriate information to EHR and data collection systems within specified time frames.
- Contact all clients after behavioral health hospitalizations to complete a Transition of Care form and uploads into HealthEC.
- Attends weekly staffing to update team on progress.
