Senior Transitions

Senior Transitions is a behavioral health program that helps older adults adapt and cope with:

  • Grief and loss
  • Changes in health or loss of well-being
  • Medication management
  • Anxiety, nervousness, or worry
  • Relationship challenges
  • Depression* - see description and screening test below
  • Aging and failing memory
  • Anger management
  • Changes in eating or sleeping patterns
  • Low self-esteem
  • Loss of  interest in activities they once enjoyed
Any of these conditions can make other health problems worse. They also rob older adults of an opportunity to enjoy their senior years. Yet all of these conditions are treatable. With treatment, many older adults find enjoyment and pleasure where there was once only hopelessness.


 
 

* When people feel sad, irritable, or withdrawn for more than two weeks, the source may be depression or anxiety. Depression is a type of mental illness where people feel sad, irritable, or withdrawn for more than two weeks. These feelings are so strong that they can disrupt normal routines and interfere with a person's ability to function. Symptoms can include:

  • Sadness
  • Loss of interest or pleasure in favorite activities
  • Recent weight gain or loss of more than 10 pounds
  • Difficulty sleeping or sleeping too much
  • Chronic fatigue
  • Feelings of worthlessness
  • Thinking of death or suicide

About Senior Transitions

The program provides more than a counselor. Patients in Senior Transitions participate in a variety of group therapy activities that not only help their symptoms, but also connect them with others who are experiencing many of the same concerns.  
 
In addition, patients are followed by a psychiatrist who coordinates care with the primary care physician, in particular to determine the most effective medications or therapies. The treatment team meets regularly to assess each patient’s progress and to adjust treatment plans when needed. Family or caregiver involvement is both encouraged and facilitated.

Up to three days a week, patients participate in group and individual therapy sessions for approximately three hours. A meal is served, providing an opportunity for fellowship and nutrition. All treatment takes place during the day, conveniently located behind Ferrell Hospital at 1408 Grant Street in Eldorado, IL. Patients return home each afternoon after treatment.
